Parent/Guardian Signature

State law under Wisconsin State Statute 118.9 requires written permission before any medication (prescription or over-the-counter) can be administered at school. All prescription medications must be administered from the original containers that are clearly marked with the name of the student, name of the drug, concentration, dose, route, instructions, physician's name, and the expiration date.


By typing my first and last name below to serve as my electronic signature, I acknowledge that school staff members are not allowed to provide ibuprofen, acetaminophen, or any other medications to students except for the office medications/devices listed on this form. I authorize the school office staff to administer the prescription medication(s) and/or non-prescription medication(s) to my student as listed and instructed on this form. I further give my permission for the school personnel to contact the prescribing physician as needed to clarify instructions or share appropriate information. I agree to notify the school of any changes to the above medication instructions.

I declare that our child has no preexisting medical conditions that have not been revealed in this form.  I agree to notify the school of any changes to the student's medical history while the student is enrolled at the school.  I understand that the school has the right to terminate enrollment of a student without a refund if the school was not aware of the student's medical condition or needs.


Should my student incur an illness or injury on or off campus that requires temporary accommodations within the school day (concussion, broken bone that requires sling/crutches/wheelchair, etc.) I will contact the school to provide the necessary information to best serve my student.

  • I certify that the emergency contacts listed here have the ability to sign the student out of school.

  • I acknowledge that in case of emergency, the School's procedure will be to contact the parents/guardians or emergency contacts as soon as possible. If unable to reach either parent, the school will contact the next emergency contact listed. Staff members will administer basic care or first aid as needed until parent/guardian is reached or until EMS arrives (if deemed necessary by staff).

  • I will not hold the Trinity Lutheran Classical High School and its partner, Elm Grove Evangelical Lutheran Church, financially responsible for any emergency care, medical expenses, or medical transportation of said student. I understand that the School does not carry medical expense insurance for the benefit of any student. I understand any costs for medical care or emergency transportation are my responsibility.

Computer Network and Acceptable Use Policy

Philosophy and Goals - Part 1

  • The internet is a network of networks used by educators, businesses, the government, the military, and other organizations. The internet can be used to educate and to inform. As a learning resource, the internet is similar to books, magazines, video, CD-ROM, and other information sources.

  • Access to the internet at the schools and offices of Trinity Lutheran High School - TLCHS is a privilege - not a right. Access entails responsibility.

  • Because the internet is a fluid environment, the information which will be available to students is constantly changing; therefore, it is impossible to predict with certainty what information students might locate. We do use a filtering device to automatically screen the sites which are called up. While our intent is to make internet access available to further educational goals and objectives as well as to apply technology in accordance with Christian principles, students may find ways to access other materials. We believe that the benefits to students from access to the internet, in the form of information resources and opportunities for collaboration exceed the disadvantages. The schools will preserve academic freedom with certain standards but, ultimately, parents and guardians of minor students are responsible for establishing and communicating the standards that their children should follow when using media and information sources.

Philosophy and Goals - Part 2

  • The networking environment requires that the TLCHS define guidelines for student exploration and use of electronic information resources. (See Internet Use Rules and Regulations Below) Such guidelines address issues of privacy, ethical use of information with respect to intellectual property, using the networks for illegal activities, or knowingly spreading embedded messages or other computer programs that have the potential of damaging or destroying programs or data.

  • Students may use the internet to participate in distance learning activities, to ask questions of and consult with experts, to communicate with other students and individuals, and to locate material to meet their educational and personal information needs. School and library media specialists and teachers have a professional responsibility to work together to help students develop the intellectual skills needed to discriminate among information sources, to identify information appropriate to their age and developmental levels, and to evaluate and use information to meet their educational goals.

  • TLCHS reserves the right to delete any files put onto the computer due to internet use.

  • TLCHS reserves the right to prioritize the use and access to the system.

  • TLCHS requires that all students have a signed Acceptable Use Policy on file.

Internet Use Rules and Regulations of TLCHS

The following are examples of actions that are not allowed:

  • Sending or displaying offensive or pornographic messages or pictures.

  • Using obscene or defamatory language.

  • Harassing, insulting, defaming, or attacking others verbally.

  • Damaging computers, or computer systems.

  • Destroying data of another user.

  • Violating copyright laws.

  • Using another person's password.

  • Intentionally wasting limited resources.

  • Using the internet for commercial purposes.

  • Interactive game playing.

  • Downloading non-school related files or programs.

  • Other uses deemed unacceptable by the general standards of our faith and our school.


In addition to the inappropriate behaviors listed above, these ground rules for use of the world wide web are also included:

  • Do not give out personal information, phone numbers, social security numbers, credit card information, or addresses.

  • Do not enter news groups or chat rooms without approval of an administrator or teacher.

  • Do not fill out questionnaires without the permission of an administrator or teacher.

  • Do not order a product over the internet.

  • If you arrive at a site that is offensive to Christians, use the back button to leave the site and report it immediately to an administrator or teacher.

Violation Consequences

Violations of this acceptable use policy will be dealt with through the discipline cycle and may result in:

  • Withholding privileges.

  • In-school suspension.

  • Out-of-school suspension.

  • Expulsion.

  • Students and their families are responsible for any financial expenses resulting from improper technology use. This includes payments to staff members for their time to correct problems due to abuse.

Authorizations

Authorization - Media and Directory


Media Authorization: I consent to have photographs and/or videos of my student to be used to publicize school programs, acknowledge students' accomplishments, for teacher review and development, and/or to illustrate educational activities. For example, my student's picture will be included in the school yearbook.


School Directory Authorization: I give permission for our family to be listed in the Trinity Lutheran High School directory. I understand the directory will be shared with our photography service, graduation service, and for school purposes only and will not be shared with other institutions or businesses.


Internet Safety/Technology Usage Policy


Over the next several years, Trinity Lutheran High School may make investments in technology as we prepare our students to be 21st century learners. Thanks to these investments, our students and staff will have access to networked multimedia computers with full-time connections to the Internet. There are computers with network access in classrooms, and the media centers (libraries). We will provide this access because of the wealth of educational resources available on the Internet. The purpose of our Internet Safety/Technology Usage Policy is to describe the appropriate uses of the School‚ computers, networks, Internet access, and other technology resources. It specifically addresses the issues described in the federal Children Internet Protection Act (CIPA): - access by minors to inappropriate matter on the Internet and World Wide Web; - the safety and security of minors when using electronic mail, chat rooms, and other forms of direct electronic communications; - unauthorized access, including so-called and other unlawful activities by minors online; - unauthorized disclosure, use, and dissemination of personal information regarding minors; and - measures designed to restrict minors access to materials harmful to minors.

As a parent or legal guardian of the student specified above, I have read, understand, and accept the Acceptable Use Policy, the Media and School Directory Authorizations, and the Technology and Internet Use Policy.


By typing my first and last name, I sign this parental acknowledgment form and I accept these policies and understand that internet access and e-mail are privileges which may be revoked, temporarily or permanently, at any time if my student does not follow these rules. As my student uses a personal computer on campus or computers owned by TLCHS, I hereby agree that my student will comply with the above stated rules and guidelines. I understand that it is impossible for staff of TLCHS to monitor or restrict access to all controversial materials. (These materials would not likely be found accidentally while doing normal research or communication; a user would have to intentionally look for them.) Should I later become concerned about what my child is reading or viewing on the internet, I understand that I can contact the headmaster of the school regarding my concerns.

School Enrollment Contract


We, as parent(s) or guardian(s), and in consideration of, TLCHS understand that according to the Final Regulations-Family Educational Rights and Privacy Act, dated June 17, 1976, it is no longer necessary to obtain written consent to release records between schools. It states those school officials, including teacher within the educational institution and officials of other school systems in which the student may intend to enroll, may receive a student’s records without a written consent for such release.  Submitting school records is a required part of the school's application process.

We, as parent(s) or guardian(s) , and in consideration of, Trinity Lutheran Classical High School - TLCHS accepting my student as named on this (re)enrollment form as a student at TLCHS school agree to make all payments for tuition and other necessary and normal charges timely and in accordance with the Tuition Agreement or your enrollment in a Choice Program.

We agree and understand that unless all payments are current, no school records regarding our student will be issued or accessible at any time. We agree and understand that unless all tuition is paid in full, no diploma or transcript or final report card will be issued. We further agree and understand that when tuition payments are in arrears more than 10 days without an approved payment plan, my student may not be permitted to attend classes. We agree that, in the event of default in the payment of any amount due, interest may immediately accrue , at the discretion of the Business Office, and be charged monthly, at a rate of 12% per annum, and if this account is placed in the hands of a collection agency or attorney for collection or legal action, to pay an additional charge equal to the cost of collection including interest, collection agency, attorney fees and court costs incurred, as well as interest.


We agree that any payments not made in full within five (5) days of the posted agreement due date will be subject to an 18% late fee and any payments made directly to TLCHS via ACH or check that are returned for any reason will be subject to a $35 fee.


The following statement applies to parent(s) or guardian(s) whose student becomes 18 while enrolled in school. Because our student will become 18 years of age during the 2025-2026 school year and will become an adult under Wisconsin law, we agree that, not withstanding this fact, we will continue to be responsible for the tuition and any other normal expenses for the continued education of our student.

We agree that we are obligated to pay annual tuition and all other charges for the entire school year and that we are not entitled to any deduction or refund from any such obligations in the event of:


  • Withdrawal of the student

  • Absence occurring at any time during the school year

  • Dismissal or expulsion of the student from the School


We understand and agree that this agreement extends until the graduation of the student(s) from TLCHS or the termination of this agreement as provided herein. Accordingly, we understand the term of this agreement shall be in effect immediately for the 2025-26 academic year and shall renew automatically for each successive academic year until graduation from TLCHS unless and until this agreement is terminated by TLCHS or written notice of termination is received by the TLCHS Headmaster. If our student(s) will not be returning the following school year, we assume all responsibility for notifying TLCHS in writing by April 28th of the current school year.

We agree that we and the student will honor and be bound by the policies and the Rules and Regulations of the School. We understand that students or families who fail to observe the policies or Rules and Regulations of the School or whose lack of cooperation impedes the ability of the School to meet its educational objectives will be asked to leave the School.


If legal restrictions are in effect, please ensure this is clearly communicated to our Office of Student Services. Alert office of persons not allowed to see student in school or persons not allowed to pick up student. Provide any pertinent legal document stating these restrictions.


This Contract, along with any additional Terms and Conditions in the Agreement, constitutes the entire agreement between the parties and may not be modified except in writing signed by the parents or guardian and the School.

BY TYPING MY FIRST AND LAST NAME BELOW, I/WE HAVE READ AND UNDERSTAND THE FINANCIAL TERMS, CONDITIONS, AND AGREEMENTS STATED ABOVE AND AGREE TO ABIDE BY THEM AS WELL AS THE POLICIES, RULES, AND REGULATIONS AS STATED IN THE SCHOOL PUBLICATIONS.
